Documentation/Billing Corrections (If Not Signing Note)
This guide provides step-by-step instructions for rendering
providers who need to make corrections to their notes.
Confirming
Claim Creation
Below are key points to review in the billing tab to confirm
that a claim has been created:
- View of the Billing
Slip: The billing slip will display the claim # for the encounter
along with the date and time it was created.
- Non-Editable Fields:
Once a claim has been created, certain billing values (e.g., Procedure
codes, Modifiers, Units, Diagnoses, and Bill) will no longer be editable.

- If a claim has been created, you will need to
reach out to billing for assistance with code corrections.
Making
Corrections to a Signed Note
If the claim has not been created and corrections to
documentation are needed, the encounter must be amended
Amending an Encounter
- Reach out to your Supervising Provider, Manager,
or Practice Coordinator to request an amendment for the encounter.

- Once the Supervising Provider, Manager, or Practice Coordinator has amended the encounter, the athena user must send the encounter to the rendering provider for review, specifically requesting the amendment.


- RENDERING PROVIDER CONTINUE STEPS BELOW
- Once the encounter has been amended, click on
the Session Note to make necessary changes.
Updating
Diagnoses Codes
- Add or update the
diagnosis code in the Session Note under the Assessment &
Plan section.

- Changes made in the
Assessment & Plan will reflect in the Billing Tab.
Updating
CPT/Procedure Codes
If the Billing Tab is Editable:
- Open the signed encounter.
- Navigate to the Billing Tab.
- Edit the Procedure code.
- Click Save.
If the Billing Tab is Not Editable:
1. Confirm if the claim has been created.
2. Follow the steps below to create a patient case and send it to billing.
Creating
a Patient Case for Billing
- Click on the Hamburger
Menu.
- Select Create Patient
Case.
3. In the case description,
provide billing with the updated CPT code.

4. Select Data Entry
and then click on Save.
5. Leave the case in review
for "Billing."
Note: Billing will follow up and update the procedure code as
needed.
