Intensive Outpatient Program - Admission Assessment Workflow
Overview: This tip sheet outlines the documentation process for patients in Geode's Intensive Outpatient Program.
Admission Assessment
The initial IOP appointment will be scheduled as a IOP Admission Assessment 60. Patients will complete the PHQ-9 and GAD-7 screenings during their self check-in and the results will auto populate into the screenings section of the session note after the appointment is checked in.
If screenings were not completed during self check-in, screenings can be completed during the session by searching for the GAD-7 and PHQ-9 in the search box.

Document the HPI, Mental Health Exam and Assessment and Plan sections of the session note.
Note: For Georgia IOP, selecting the GA Initial Therapy Encounter (Non Prescriber) reason for visit will add in documentation to complete in the HPI and A/P sections.
Treatment Plan
A Treatment Plan will need to be created to define the patient's concerns, assess their characteristics, and enter goals to measure outcomes as treatment progresses.
Select the "Care" chart tab and click the doble arrows to open a Treatment Plan.
Under the Treatment Plan tab, select Add Treatment Plan
Add Concerns and Client Characteristics as appropriate. Goals can also be added to Treatment plan. Clicking the plus sign will allow a custom goal to be created, or select "Apply Template" to choose from a templated list of goals.
Once the treatment plan is finalized, update the Date of Next Review in the Plan Summary.
Lastly, send the Treatment Plan to patient and Supervising or Collaborative Geode Provider (if appropriate) for signature.
Click the plus sign next to Attestation Artifacts
A PDF preview of the Treatment Plan will appear first. Scroll to the bottom to send for signatures. The rendering provider’s name is pulled in by default. Use the available fields to send the plan to the patient and relevant Geode providers for attestation.
