High Symptom Severity Checklist

Criteria for a Patient Case to generate:


  1. Patient has a PHQ-9 of 15+ (Either imported from our check in vendor or manually entered in the “Screening” section of the exam).

  1. Positive result of PHQ-9 question #9 “Thoughts that you would be better off dead or of hurting yourself in some way”

  1. Selecting “Yes” for “High Acuity Patient” in Past Medical History

 

·     When will a patient case generate?:

  1. Patient case will generate 15 minutes after patient has been checked out of appointment. 
  2. A chart alert will also generate 15 minutes after checkout to help identify high acuity patients when their chart is first opened.
  3.  A screenshot of a message

AI-generated content may be incorrect.
Note: Selecting Yes for High Acuity Patient in Past Medical History will continue to trigger a patient case every 90 days until the response is updated to No.

If the patient no longer meets high acuity criteria, be sure to change the response to No to prevent repeat case creation.


Workflow steps:

  1. Review patient case
  1. Enter X in the corresponding checkbox in the patient case:

  1. Select leave in REVIEW to your care coordinator (under [state abb]carecoordinator for assistance. Save.

To find the case in the chart:

  1. Select the Find Tab under chart view
  2. Select Type > Patient Case > Clinical Question
    1. The case will be saved under "High Symptom Severity Checklist"
    2. Please note, there are multiple ways you can find the patient case including under Find > Time tab and under the Visits tab.
