High Symptom Severity Checklist

Criteria for a Patient Case to generate:


  1. Patient has a PHQ-9 of 15+ (Either imported from Epion or manually entered in the “Screening” section of the exam).
  1. Positive result of PHQ-9 question #9 “Thoughts that you would be better off dead or of hurting yourself in some way”

  1. The Past Medical History entry for “High Acuity” Y/N of “Yes

1. Select the History tab in the chart.

2. Select the chevron to the right of Past Medical History.

3. Select “Yes” for High Acuity.

 

 

 


When will a patient case generate?:

  1. Every Monday for patients checked out within the last 7 days with one or more of the above criteria met.
  2. Patient case will be generated once every 90-days as long as the patient meets the criteria for high symptom severity.
  3. Patient case will generate for the first seen provider in the previous week if the patient is enrolled in both med mgmt. and therapy at Geode.
  4. Patient case will be suppressed if the patient has been seen for an advanced treatment within the past 7 days. 

Workflow steps:

  1. Review patient case
  1. Enter X in the corresponding checkbox in the patient case:

  1. Select leave in REVIEW to your care coordinator (under [state abb]carecoordinator for assistance. Save.

To find the case in the chart:

  1. Select the Find Tab under chart view
  2. Select Type > Patient Case > Clinical Question
    1. The case will be saved under "High Symptom Severity Checklist"
    2. Please note, there are multiple ways you can find the patient case including under Find > Time tab and under the Visits tab.
