                                                                  Discharge Letters


The following Discharge Letters are available to select from when discharging a patient in either the Encounter Sign Off screen > + Letter or Order Group > + Letter:  

o   Discharge Letter-Discussed with patient:

·       Select this letter when you have discussed the discharge process with the patient. 

o   Discharge Letter-Unable to Reach:

·       Select this letter when you need to discharge the patient and are unable to reach them prior to sending the letter. 



·       Select Add on the appropriate letter. 

·       Select View or Edit to Edit the letter. This step is essential as there are templated option to select from within the letter. 


                                                              

·       Click inside the letter to make edits. Select the appropriate options. 


                                                                                                    

·       Select View actions to close the letter. Send through the patient’s preferred method of communication and certified mail.

Notify the Practice Assistant of the discharge reason. Discharge reasons will be documented on the patient’s Quickview under “Med Mgmt Discharge Reason” and/or “Therapy Discharge Reason” 




If the patient will not return to Geode for an undischarged service line, mark the patient as Inactive in the Quickview.



Do not inactivate the chart until the notice period listed in the letter has passed.
