Typically, in an outpatient mental health setting, AMAs are used when a patient declines a higher level of care.
Typical Uses and Examples:
o Adverse Medication Reaction (TMS, Spravato BP issues, or other treatments, or medications that noted side effects are happening, and the patient refuses to be transported to the ER either by family member or by 911)
o Injury- patient falls or had injury on site or causes injury to themselves and declines emergency treatment.
o Crisis
- patient is in clinic and is exhibiting increased risk of harm to themselves or others and it is recommended that they seek evaluation for psychiatric hospitalization but refuses to go.· Select Add on the appropriate letter.

Click inside the letter to make edits. Select the
appropriate options.
· Select View actions to close the letter. Send through the patient’s preferred method of communication.
Notify the Practice Assistant of the AMA. Discharge reasons will be documented on the patient’s Quickview under “Med Mgmt Discharge Reason” and/or “Therapy Discharge Reason”
If
the patient will not return to Geode for an undischarged service line, mark the
patient as Inactive in the Quickview.
